Welcome to Hillsboro Parks & Recreation programs! When completing this form, we request as much information as you feel comfortable providing - requested information is confidential. Inclusion support information helps Parks & Recreation staff create a meaningful, tailored, and inclusive experience for participants. A support request must be completed before inclusion services can be provided.

Based on information provided in the Inclusion Support Request, staff will create an Inclusion Success Plan that will be implemented during enrolled program or activity. The Inclusion Success Plan will be shared with inclusion staff assigned to work with participants and supervisors of enrolled programs.
Inclusion Support
If a participant requires support to be successful in a program, the City may provide interpreters, inclusion assistants, or tools for accommodation free of charge. Participants are welcomed to provide their own interpreter or aide, personal support worker, or direct support person at no additional cost to the participant. Personal aides and interpreters may be asked to complete a background check. Depending on the nature of the program, parents/guardians may not be allowed to participate as a personal aide.

If the assigned aide or interpreter becomes unavailable to assist and a substitute aide or interpreter cannot be found, the participant may be withdrawn from the program and a full or partial refund will be given for fees paid.

Personal Care
Hillsboro Parks & Recreation does not provide personal care, including but not limited to: toileting, grooming, transferring, feeding, administering medication, etc.

Unsafe Behaviors
Actions threatening the safety and well-being of yourself, or others will not be tolerated. Verbal or physical intimidation, harassment, or abuse may result in suspension of facility use or removal from a program.
